Terms of Service
Effective November 5, 2026 · Fairline Health LLC, West Palm Beach, Florida
Draft. This document is under review by our healthcare attorney and will be finalized before we accept our first patient. Nothing here is a substitute for that review.
These terms cover your use of fairlinehealth.com and the Fairline Health patient portal. Medical care is provided separately by independent licensed providers under their own consent, which you accept during the health assessment.
1. Who we are and what we do
Fairline Health LLC ("Fairline," "we") operates this website and patient portal, handles billing, support and shipping coordination, and contracts with independent healthcare providers and a licensed compounding pharmacy on your behalf. Fairline does not practice medicine, does not employ the providers who review your case, does not compound or dispense medication, and does not decide whether you receive a prescription.
2. Eligibility
You must be 18 or older, live in a state we serve, and provide accurate and complete information in the health assessment. Providing false information can result in harm to you and ends our relationship.
3. Medical care
Prescribing decisions are made solely by licensed providers through [MEDICAL GROUP NAME]. Completing the assessment does not guarantee a prescription and does not create a provider-patient relationship until a provider accepts your case. The telehealth consent you sign governs that relationship.
4. Medication
Prescriptions are filled by Olympia Pharmacy, a licensed compounding pharmacy. Compounded medications are not FDA-approved. The FDA does not review compounded drugs for safety, effectiveness or quality. Use medication only as directed by your provider.
5. Payment and subscriptions
Prices are shown before you pay. Your first payment is charged when you choose a plan; later payments are charged monthly on your renewal date until you cancel. Refunds and cancellation are governed by our Refund and Cancellation Policy, which is part of these terms.
6. Your account
Keep your sign-in private. You are responsible for activity on your account. Tell us at once if you believe it has been accessed without permission.
7. Emergencies
Our service is not for emergencies. If you are having a medical emergency, call 911.
8. Acceptable use
Don't use the service for anyone but yourself, don't resell or share medication, and don't interfere with the site or other people's accounts.
9. Limitation of liability
To the extent permitted by law, Fairline's liability to you for claims arising from this website or your subscription is limited to the amounts you paid us in the twelve months before the claim. Fairline is not liable for the clinical decisions of independent providers or for medication prepared by the pharmacy; those parties carry their own responsibility and insurance.
10. Changes and governing law
We may update these terms; we'll post the new effective date here and email you about material changes. Florida law governs these terms. Disputes are resolved in the state or federal courts of Palm Beach County, Florida.
